Adjunctive tenotomy during middle ear surgery.

چکیده
INTRODUCTION We describe indications and surgical technique for dividing the stapedius and tensor tympani tendons during middle ear surgery. Middle ear muscle reflexes involve contraction of the tensor tympani muscle (TTM) and stapedius muscle (SM) to increase impedance of the middle ear, and may shield the inner ear from loud, continuous nonimpact noise. Sectioning SM and TTM tendons has been advocated for a number of conditions, including improving access to anterior epitympanic (supratubal) recess, releasing a medialized malleus during tympanoplasty and ossiculoplasty, tensor or stapedial myoclonus, reflexive vertigo, and Meniere disease.

Endoscopic Middle Ear and Mastoid Surgery for Cholesteatoma
Introduction: To reduce incidence of residual cholesteatoma following ear surgery; and to reduce the need for second look “open” mastoidectomy using endoscopic mastoidotomy. Ten-year retrospective chart review of 249 primary cholesteatoma cases (1994-2004) with a minimum follow-up of two years. The first objective was to evaluate the effectiveness of otoendoscopy in reducing the incidence of “...
متن کاملFeasibility of Endoscopic Treatment of Middle Ear Myoclonus: A Cadaveric Study
Stapedius and tensor tympani tenotomy is a relatively simple surgical procedure commonly performed to control pulsatile tinnitus due to middle ear myoclonus and for several other indications. We designed a cadaveric study to assess the feasibility of an entirely endoscopic approach to stapedius and tensor tympani tenotomy. We performed this endoscopic ear surgery in 10 cadaveric temporal bones ...
متن کاملEndoscopic Treatment of Middle Ear Myoclonus with Stapedius and Tensor Tympani Section: A New Minimally-Invasive Approach
Aims: We describe a new, entirely endoscopic surgical technique for treatment of middle ear myoclonus. Case Presentation: In our patient, the stapedius and tensor tympani tendons were sectioned to control chronic middle ear myoclonus. The procedure was performed using endoscopic ear surgery techniques, with the aid of rigid Hopkins rod endoscopes. Control of the pulsatile tinnitus was achieved ...
متن کامل[The modern view of the clinical significance of tensor tympani muscle].
The objective of the present work was analyse the modern views of the role of tensor tympani muscle in the development of otic pathology. In this context, the authors discuss tenotomy of the middle ear muscles for the treatment of Meniere's disease, diagnostics and clinical aspects of middle ear myoclonus, and tonic tensor tympani syndrome.
متن کاملMiddle ear myoclonus: a new technique for suppression of spontaneous clicking tinnitus.
Middle ear myoclonus is one of the causes of clicking tinnitus and can be psychologically distressing. Current management of intractable clicking includes medication or tenotomy. Two cases with spontaneous intrusive irregular clicking are presented where relief and a sense of control were obtained using non-invasive self - administered zygomatic pressure. This technique may be useful in selecte...
